Required paperwork for participation in LandSea

  • LandSea Registration (& deposit)
  • LandSea Release & Waiver
  • Pre-participation Physical

The forms MUST be submitted no later than July 15th. Failure to meet the deadline may result in your ineligibility to participate and forfeiture of any refund.

Online forms:

  1. LandSea Registration
    First things first, let's get you signed up for the trip! After completing the registration there will be a link provided to complete the $100 non-refundable deposit by credit card. If you cannot find the e-mail with the link, please contact us and we will re-send it to you.  
    • Complete the Scholarship Application (if applying for a need-based scholarship). In order to be considered for a need-based scholarship, your registration, deposit and scholarship application must be submitted by May 15.
  2. LandSea Release & Waiver
    Before signing the Participant Agreement, please review the Participant Handbook

The following forms need to be printed and returned to the New Student Forms address below:

  1. Pre-participation Physical
    A doctor's physical is required of all students before participating in LandSea or Athletics. Please print out the form and bring to your physician.
  2. Outfitter's Release (please choose one):
    --Outfitter's Relase (over 18)--
    --Outfitter's Release (under 18 Form with parent/guardian signature)--
    The program rents canoes from Raquette River Outfitters in Tupper Lake, NY who require this release for use of their equipment.
  3. Rafting Release
    Adirondack Rafting is the company LandSea uses for rafting. The Adirondac Rafting Company requires this release for participation.
  4. Proof of Insurance (copy of insurance card):
    Make a copy or scan of both sides of your insurance card. Please check to make sure the copy/scan is legible! If using a smartphone to take a picture of your insurance card, please use a scanning app (i.e. CamScanner). This provides us a cleaner image of your card. 
    • If you are enrolling in the College's insurance, please enroll in the program by our July 15 deadline. Once enrolled, you can download your insurance card and send a copy to us. Here are instructions for downloading your college insurance card: College Insurance Download Instructions

Required by the Student Health Center:

  1. Medical Questionnaire
    All Kalamazoo College students are required to complete the medical questionnaire prior to arriving on campus.  You will need your "K" user name and password to access the site.
          - If you are under 18, at the end of the medical questionnaire will be a Minor's Consent to Treat form. 
  2. Immunizations
    All Kalamazoo College students are required to submit their immunization records through the Student Health Link website. As a new student, you'll need to create an account the first time you visit this site.

Note: In the run-up to LandSea the Student Health Center is operating on reduced summer hours and response to your phone call or e-mail may be delayed. Because of their reduced hours the processing of forms may take several days, particularly immunization records which require additional review and approval. 

Please remember that all of the above forms must be submitted no later than July 15th

We highly recommend that you send in your forms as soon as you have finished them.  The earlier, the better. 

To check the status of your completed forms

Consult the New Student Portal. Go to the right column of the web page and click WebAdvisor for Students - Communication - Status of My Documents. It usually takes a few days to process paperwork, so there might be a slight delay!

 

International Students

Please check in with the Center for International Programs about any additional requirements or deadlines.

 

A note about health insurance coverage:

Not every health insurance plan will cover medical expenses incurred out of the state of residence. Other companies will only allow emergency coverage. Please check with your insurance company about their travel coverage and ensure that your policy covers you during your time in New York. Many companies will allow travel exemptions for a short period of time if they are made aware ahead of time of the travel to another state.
